In 2018, General Data Protection Regulations (GDPR) were established which require me to notify you about the storage of your personal data. This personal data is obtained to ensure there are no contraindications for treatment and to work with you in the context of your personal history.
You can, of course refuse to provide this information, however, I will be unable to provide treatment, as this form is a legal requirement for my insurance.
How long will my data be stored?

For adults, 5 years after the date of your last treatment.

For children 5 years after they turn 18.
Where and how will my data be stored?

Client intake forms and treatment notes will be stored on paper only in a locked container in my home.

Contact information (name, phone number and email address) will also be kept on my phone to enable communication with you. My phone is password protected.
Who can see my data?

I am the only person with access to your data. I will never share your data with anyone, unless I have express written consent from you.

You have the right to see and request amendments to your personal data, including my treatment notes at any time.
How is my data used?

I will only use your contact information to send you reminders for appointments, or to book or follow-up with you after treatment.

I will refer to your personal history and my notes before each appointment to ensure your treatment remains relevant, safe and on track with any goals you may have.
